Mental health eligibility requirements

We are a state-certified mental health clinic that offers affordable counseling services to individuals and families in surrounding areas who:

  • Have a household income under 200% of the Federal Poverty Level
  • Do not have any primary health insurance (sliding scale fee based on income will be charged)
 
We do not provide services to patients whose income is above 200% of the FPL.   
Family size is considered self, spouse, and any dependents you claim on your tax return. 
If you feel you may qualify for our services, please call Church Health Services at
920-887-1766 option 2. Counseling is available at our offices in Beaver Dam.

Please bring proof of household income (pay stubs, unemployment compensation statement, and/or previous year's tax return) with you to your first appointment.

We Accept Wisconsin State Medicaid Insurances

At this time, we accept Wisconsin Medicaid and certain Wisconsin Medicaid HMO insurances. If you are unsure of the type of Medicaid insurance you have, or any questions about the types of insurance we accept, please reach out to us at (920) 887-1766, option 2.
