In order to qualify for our Medical Services, a person must meet the following guidelines:
1. They cannot have private health insurance.
2. They must be between the ages of 18-64.
3. They must meet our income guidelines as shown below.
4. A $30 visit fee must be paid at time of service.
We do not provide services to patients whose income is above 200% of the Federal Poverty Line (FPL). Call us at (920) 887-1766, option 1 for current income levels.
Please bring proof of household income (pay stubs, unemployment compensation statement, and/or previous year's tax return) with you to your first appointment.
