Our dental clinic offers affordable preventative and restorative hygiene services to families and individuals of all ages in surrounding areas who:
- Have a household income under 200% of the Federal Poverty Level
- Do not have any primary dental insurance (sliding scale fee based on income will be charged)
We do not provide services to patients whose household income is above 200% of the FPL. Family is considered self, spouse, and any dependents you claim on your tax return. If your income is below 200% of the FPL and you believe you may qualify for our services, please call us at (920) 887-1766 option 1.
At your first appointment, please bring your proof of household income (pay stubs, unemployment compensation statement, and/or previous year's tax return) with you.
