Service Area

The service area of CHS includes most of Dodge County plus an eastern portion of Columbia County and the southeastern corner of Green Lake County. The service area does not include the city limits of Columbus, Watertown or Waupun. People living outside the designated service area may apply for care if they an established long-term relationship with a physician on the active medical staff of Beaver Dam Community Hospital.
